I have read it as taking into account that some software ships (like shrink-wrap boxes) while some is implemented locally (like deployed to production server). Both are business decisions, driven by other factors than whether a software increment was completed in a Sprint, so for me this question was clear.

a- When it makes sense.
b- To make sure the Team is done every Sprint.
c- Without exception.
d -Whenever the increment is free of defects.

I think the question relates to an understanding of 'potentially shippable' (for an increment at the end of the Sprint). 'Shippable' holds that only 'Done' Product Backlog items should be in the increment being reviewed at the end of the Sprint. 'Potentially' because it's up to the Product Owner to decide on the overall value of the increment whether to release it. 'Definition of Done' should assure that, when Product Owner decides to release, no additional work needs to be performed.
So, it not so that without exception every increment is shipped.

I think the terminology is a little confusing here. It seems to be saying, "The Product Owner must do one of the of the following in each sprint, either release a product implement, or implement the tasks committed to by the team for that sprint." It seemed like it was making a distinction between releasing an increment to the customer, or completing the stories assigned to the increment (possibly without an actual release). If that's the case, it seems that one or the other is always necessary. Clearly a production release is not always necessary, it could be a release to a UAT site as part of a larger release plan. I think a little more explanation would help the question set the context a little better.
