Where can I find Team maturity survey for Scrum Team

Hello

Could somebody recommend where I can find Team maturity survey for Scrum Team? As I've read about this assessment it should also include Process, QA and other estimations. Could you lead me right way?

The whole point of Scrum is to produce a Done, valuable, and immediately usable Increment each and every Sprint.

If empiricism is so obfuscated we need a survey to compensate for the lack of transparency, we already have enough to deal with right there.

Not sure where you have read about this survey but I would question the quality of the information you have found.  I've never used a team maturity survey with a Scrum Team.  It should be pretty obvious if the team is maturing by the ability to consistently deliver valuable usable increments that meet the stakeholders needs. 

Where have you read about the need for this survey?

I watched in YouTube that's recommended to do it to learn at what level the team is now. They say that this way can be tested different levels e.g. Scrum processes, testing, product understanding and others
